A clinical presentation of this condition occurs at a mean of 7.2 weeks after the last regular menstrual cycle, within a range of 5 to 8 weeks. Later presentations are more common in communities that are deprived of modern diagnostic ability.

The early signs of an ectopic pregnancy include: Inflammation and pain in the lower abdomen; Mild discomfort and mild pain, and; mild vaginal bleeding. An ectopic pregnancy is usually described as a failing pregnancy and withdrawal bleeding can be caused by the falling levels of progesterone from the corpus luteum which is on the ovary. This may be indistinguishable from the implantation breed of a normal early pregnancy or an early miscarriage, and; Pain during a bowel movement.

Patients who have late ectopic pregnancy usually experience bleeding and pain. This bleeding will be both internal and vaginal and has two distinct path physiologic mechanisms: External bleeding as a result of the falling progesterone levels, and; Internal bleeding as a result of hemorrhage from the affected tube.

The differential diagnosis at this point is between early normal pregnancy, ectopic pregnancy, and miscarriage. Pelvic infection is virtually ruled out by the presence of a positive pregnancy test and it is in fact rare indeed to find an early ectopic pregnancy with an active Pelvic Inflammatory Disease (PID). However, PID is the most common misdiagnosis that is given to early ectopic pregnancy.

More severe internal bleeding can result in various types of pain including: Pelvic pain, abdominal pain; Lower back pain, or; Shoulder pain. This is caused by the irritation of the diaphragm caused by free blood being tracked up the abdominal cavity which is an ominous sign; there may be tenderness on one side of the pelvis or cramping, and; the pain is of recent onset, meaning it must be differentiated from routine cyclic pelvic pain, and often is getting worse.

Ectopic pregnancy can mimic symptoms of other diseases which include; Appendicitis, Pelvic inflammatory disease problems; Urinary system problem; Gynaecologic problems, and other gastrointestinal problems.
